About Your First Visit

Your first visit with the doctor will serve as a consultation. The doctor will review your mammogram films (past and present), reports, and perform a breast exam. You will be given the opportunity to ask any medical questions or voice any concerns you may have at this time.

If surgery is deemed necessary, we will have you meet with the nurse following your appointment with the doctor. The nurse will go over the pre- and post-operative procedures and schedule you for any test that may be required prior to surgery. Knowing that your time is valuable, we will make every effort to accommodate your schedule in the best possible way.

What to Bring to Your First Visit

1) FORMS: Download, print and complete the PDF forms to the right on this page. Please include all of your doctor's full names, address, phone, fax and specialty.

2) INSURANCE: You must present your insurance card at the time of your appointment. Our office will make a copy of the card for our records so that we can file on your insurance. We cannot file your insurance without your card.

3) REFERRAL NUMBER: If insurance requires a referral number from your primary care doctor it is your responsibility as the patient to obtain this number and forward it to our office before your appointment date. You may have the number faxed to our office at 214-828-2089. If you are unsure if your insurance requires a referral number or want to know what a referral is, please contact your insurance carrier. They can provide a detailed explanation.

4) MAMMOGRAMS: You will need to pick you your mammogram and/or sonogram films prior to your appointment. If you had films done at the Baylor Hospital Dallas Location (now known as Darlene G. Cass Women's Imaging Center) they will need to be notified 48 hours prior to picking up your films. They can be reached at 214/820-9232.

All films must have a written report with them. Missing reports will increase your waiting time to see the doctor while we try to retrieve them.

Please check the folder when you pick up the mammograms and/or sonogram films making sure that the current films are included as well as any prior years.

The doctor will not be able to see you without your x-rays.
